Issuance Order
Outpatient Patients
Apply at the administrative department. Register at the medical department. Apply to the attending physician and complete the form. Collect the certificate at the certification counter and pay the issuance fee.
Admitted/Discharged Patients
Apply at the administrative department and complete the application form. Confirm the required documents. Receive notification of document issuance (via SMS). Collect and pay for the document at the certification counter. (For admitted patients, the issuance fee is included in the admission fee.)
In accordance with Article 15 of the Enforcement Rules of the Medical Act regarding the retention of certificates such as diagnosis certificates and medical opinions, the retention period is three years. Certificates that have exceeded their retention period cannot be reissued.
